These forms are required for new and existing students to enroll in the 2009 - 2010 school year.

Which forms do I need to complete?


New Enrollments

For new enrollments, you will need to complete the Application for Enrollment, the Enrollment Agreement for the grade your child will be attending, and the Emergency Medical Forms. You will also need to provide Documentation of Immunization on the blue Florida form available from your child’s physician or your local health department. A copy of your child’s Birth Certificate, or Baptismal Certificate if they are Greek Orthodox, will also need to be kept on file.

Re-Enrollments

Current students re-enrolling for the 2009-2010 year will need to complete the Application for
Re-Enrollment, the Enrollment Agreement for the grade your child will be attending, and the Emergency Medical Forms.

FACTS Tuition Payment Plans

FACTS tuition payment plans offered at St. Nicholas alleviate the stress of managing education costs by distributing tuition payments over time. This service allows parents to pay for tuition on a monthly, quarterly or a convenient payment schedule that works for you.

Once you determine how much your child’s annual tuition will be, you pay the school registration fee, then you apply the balance to your FACTS account to be paid in either 1, 2, 4 or 10 monthly payments. FACTS automatically withdraws monthly payments from your checking or savings account, and in turn deposits the funds in the schools bank account.

If you qualify for a McKay or Step Up for Students (Florida Pride) scholarship and it does not completely cover your tuition costs, you can apply the balance to a FACTS payment plan.

The FACTS enrollment fee if paying either 10 or 4 months is $38.00; $15.00 if paying over 2 or 1 payments.

FACTS enrollment forms are available at the school administration office, and take less than 5 minutes to complete. Please bring a voided check to attach to the FACTS enrollment form.
